Presenter Application Requirements
- Submit this Application Form
- Identify the TITLE of each presentation
- Provide a 50 – 125 WORD DESCRIPTION for presentation
- Identify any equipment used or needed in presentation (i.e. audio/visual needs).
- Provide a 30-50 WORD BIOGRAPHY.
- Provide your RESUME and/or CURRICULUM VITAE featuring complete educational background, certifications, and all professional, industry-related experience.
- Provide your and/or all presenters' full name(s), mailing address, phone & fax numbers, and email address(es).
- Application must be submitted by December 16, 2011.
